What is multidisciplinary care?

Multidisciplinary care is used when a patient has complex needs involving multiple specialties. A team of specialists works together to create a single, unified plan, evaluating all aspects of the patient’s condition at once, ensuring every care team member knows their role and streamlining appointments, tests and other care for families.

The clinics below represent many of our multidisciplinary programs. Some clinics accept referrals from community providers, while others are designed for patients who are already receiving specialized care through OSF Children's Hospital of Illinois.

Clinics Accepting Referrals

These multidisciplinary clinics are available to children who may benefit from specialized evaluation and coordinated care. Referrals can be made by your child’s primary care provider, specialist or other qualified health care professional.

Specialty Follow-Up Clinics

These clinics provide ongoing multidisciplinary care for patients with specific conditions or who have received specialized treatment through OSF Children's Hospital of Illinois. Participation is typically coordinated through your child's existing care team.

What to Expect at Your Multidisciplinary Clinic Visit

Before your visit, our team will help you gather medical records, test results or paperwork so you don’t have to manage it all on your own. We recommend bringing a list of your child’s current medications and any questions you’d like to discuss. Children are encouraged to bring comfort and sensory support items from home that might help them feel more comfortable during their appointment. Comfortable clothing is best, and our team will let you know ahead of time if your child may need to change into a gown for any part of the exam.

Your child’s care will all happen in one location, during one coordinated visit. Your first appointment will be longer than traditional specialty visits to allow time to meet the full care team involved in your child’s treatment. Together, they’ll review medical history, discuss concerns, complete evaluations and consult with one another in real time so nothing is missed and nothing is duplicated.

You’ll leave your visit with a clear, unified care plan and next steps already in place. Your follow-up visits will build on that plan as your child grows, with the same team guiding care, tracking progress and adjusting support along the way. Our goal is for families to arrive feeling prepared and leave feeling confident and supported.
