Virtual Support for Pregnancy

From eight weeks pregnant through six weeks after birth, postpartum and pregnancy support through OSF OnCall Connect helps you stay connected to prenatal care. Postpartum is the recovery period after delivery. This program provides remote pregnancy monitoring and virtual prenatal care through a supportive virtual care team, available 24/7. It provides an additional layer of support, but doesn't replace your traditional OB care.

How Virtual Pregnancy Support Helps You

OSF OnCall Connect provides virtual pregnancy support through easy chat check-ins, monitoring and education. Your virtual care team helps you track changes in your body, get answers quickly and stay confident throughout pregnancy and postpartum recovery.

Pregnancy and Postpartum Care Services

Secure chat check-ins let you message your pregnancy and postpartum care team, get guidance, track vitals and receive support between OB visits.

  • Weekly pregnancy education and guidance
  • 24/7 access to OB-trained nurses and advanced practice providers
  • Blood pressure monitoring
  • Breastfeeding and feeding support
  • Screening for depression and anxiety
  • Recovery support, as determined by your delivery
  • Help with transportation, food or housing needs
  • Assistance ordering necessary equipment

Mobile Pregnancy and Postpartum Support

The OSF OnCall Connect pregnancy and postpartum support program, in partnership with Meridian Health Plan of Illinois and the Centene Foundation, introduced a mobile clinic to bring care directly to patients who are struggling to connect to in-person care. If eligible, the mobile clinic can visit you between appointments from eight weeks of pregnancy through six weeks postpartum. Your care team can determine if this service is right for you.

Once you are enrolled in the OSF OnCall Connect pregnancy and postpartum program, you will have access to services on the van as determined by your care team.

Eligibility

  • Be at least eight weeks pregnant or no more than four weeks postpartum
  • Have Medicaid coverage
  • Live in Illinois (Bureau, Henry, Knox, LaSalle, Marshall, Peoria, Putnam, Stark, Tazewell, Warren and Woodford counties)
  • Be enrolled in the OSF OnCall Connect pregnancy and postpartum program
