This safe, effective and proven hospital-at-home program gives you in-person and virtual care with access to your care team 24/7, just like you would receive in the hospital.

Am I eligible for OSF OnCall Digital Hospital?

When you're in our care at an OSF facility, an OSF OnCall Digital Hospital provider will evaluate several factors to determine if you are eligible for hospital care at home. These factors include your diagnosis, available support system at home, distance from the hospital and insurance coverage.

What kind of equipment will I get?

During your care, you will receive a computer tablet for virtual care video visits with your care team, a personal emergency response bracelet, vital sign monitoring devices and other supplies as your health needs require. Your care team will show you how to use all of these devices.

What if I don’t have internet or there’s a power outage?

You will be provided a telephone that connects directly to your care team, a router or hotspot for internet access and a backup power supply in case of an outage.

What if I need labs or imaging?

Lab tests, IV therapies and mobile imaging like ultrasounds and X-rays can be performed in your home. All the arrangements will be made by your care team. If you need to access these services outside your home, transportation will be provided.

How do I set up the equipment?

Our care team does the set up for you. We will make sure you understand what everything is and how it’s used.
